NOTICE TO NEW ALCOHOL APPLICANTS REGARDING BACKGROUND CHECKS – Applicants downloading paper application should be aware that it may include outdated information related to background criminal history procedure. Since April 17, 2024, In lieu of submitting directly to the ATC a full set of fingerprints and background fees, Applicants may complete prints with INDENTOGO by scheduling an appointment online by visiting https://uenroll.identogo.com. The Notice to Conduct On-Site Sampling must be submitted to ATC-SamplingNotification@atc.la.gov at least 7 days prior to conducting any on premise sampling. By completing this form you agree to follow all rules and regulations in regards to samplings, as decribed, in Title(s) 26 of the Louisiana Revised Statutes and/or 55 of the Louisiana Administrative Code.

“Pursuant to R.S. 26:306, prior to the sale of any malt beverage in a keg that is intended for consumption off-premise, the purchase is required to provide a legal form of identification, complete the “Retail Keg Registration Form,” require the purchaser to sign declaration, and provide a completed copy of the form to the purchaser. Retailer is required to maintain a record of the completed form for minimum of 6 months from the date the keg is returned to the dealer.” Completed forms are required to be made available to the ATC upon request.
